Gotta Glow Spray Tanning Consent Form


Please read and understand the following:

Spray tanning is accomplished by application of a solution containing the active ingredient: DHA (Dihydroxyacetone). DHA is generally considered to be safe and has been FDA approved ONLY  if you follow guidelines to protect mucous membranes. The FDA advises asking the following questions when considering the application of DHA products by spraying or misting:

  • Are you protected from exposure in the entire area of the eyes, in addition to the eyes themselves? (goggles or stickers can be requested if needed)

  • Are you protected from exposure on the lips and all parts of the body covered by mucous membrane? (lip balm or spa garments are available if requested)

  • Are you protected from internal exposure caused by inhaling or ingesting the product? (ventilation used as needed, and nose filters are available upon request)

If the answer to any of these questions is "no," you are not protected from the unapproved use of DHA. You should request measures to protect your eyes and mucous membranes and prevent inhalation.


What to expect: You will enter a private tent where you will remove your clothing. This is up to you and your level of comfort. You should wear a snug dark swimsuit or underwear. Spa undergarments may be available upon request. The solution will wash out of most clothing. It is always best to wear dark loose fitting cotton clothing. It is advised to wash the undergarment or clothing worn as soon as possible after your session.


Next, you will be sprayed. This process will take approximately ten to fifteen minutes. After spraying, your skin should be dry before putting your clothes back on and you should not bathe    or sweat excessively for four hours. The solution will give you an immediate bronzing effect.    The bronzing effect is a result of a temporary coloring additive in the solution that will remain on the skin until you shower. When you shower, the coloring will come off to reveal your sunless DHA tan beneath.


All people, all skin, is different. All ingredients used in this procedure are intended for cosmetic use and generally regarded as safe. There are, however, occasions where individuals may be allergic to one or more ingredients in the spray tan solution. Please read the ingredients list if you have any known allergies. If unsure, a small patch test can be done and watched over a 48 hour period. Please discuss with your technician.


Be advised there is a small percentage of people whose skin may not react favorably to spray tanning. For this reason, we do NOT advise being sprayed for the first time when your appearance is critical; (wedding/special occasion/prom) Please schedule a practice session at least 30 days before your event, for best results.


  • Common Sense Caution – Pregnant or nursing women should consult their physician, and obtain a written release before using. Spray tanning is not normally contraindicated, when a mask or nose filters are used. But each pregnancy is different, your medical care provider may prefer a more cautious approach based on your specific needs and health concerns. Those with skin conditions or auto-immune disorders should not try spray tanning.


  • Warning – This product does not contain a sunscreen and does not protect against sunburn.


Repeated exposure of unprotected skin to U.V. Light may increase the risk of skin aging, skin cancer and other harmful effects to the skin even if you do not burn.


UPDATED COVID-19 INFORMATION: Gotta Glow is following guidelines set forth by the CDC and the WHO for the health and safety of all clients. Your health and safety has always been our priority, and we will continue to make it our standard of practice. It is part of our regular protocol to practice good hygiene regularly, and for our spray tan artists to wear a surgical mask and medical grade gloves during appointments. We always offer sticky feet, disposable thongs and bras, lotion, hair nets, nose plugs, goggles, and masks, and will continue to offer these items. It is our regular standard of practice to thoroughly clean and sanitize all of our spray tan equipment and tent before and after each appointment, and we will now be adding in using EPA approved disinfectants such as Clorox or Lysol wipes and/or spray. We will be practicing social distancing, and there will be no physical contact between client(s) and sprayer with our new service (including taking payments beforehand at the time of booking). All clients must be free of all signs and symptoms of the corona virus before booking an appointment and ensure that no one in their home is sick or has any underlying health conditions. Any client with signs or symptoms of the corona virus will be declined a booking at this time. By agreeing to this document, you are certifying that you do not have any symptoms of the corona virus such as: fever, shortness of breath, extreme exhaustion, non-productive cough, diarrhea, vomiting, muscle pain, loss of sense of smell and/or taste. You are also certifying no one in your home is sick or has any underlying health issues. Since sixty percent of cases show no symptoms, as a precaution we are requiring all clients to wear a mask covering their nose and mouth. All clients must put their mask on prior to our spray tan artist’s arrival and wear it for the duration of the appointment (with the exception of face spray if a client requests it). If you do not have a mask, one will be provided to you. If you choose to have your face spray tanned, we are asking all clients to go into a separate closed room right after while our spray tan artist packs up to leave. These precautions are for the health and safety of both parties. The client also has the option to choose not to leave the room, but if they do, we are asking them to place the mask over their face right after. Doing so could potentially cause the tan to rub off slightly which Gotta Glow will not be responsible for how it may come out if this should occur. The other option would be to decline having their face spray tanned. 


I understand that Gotta Glow and their spray tan artists will not be liable or responsible should myself or anyone else in my party or home become ill or develop signs or symptoms of COVID-19. I have been provided with spray tan care instructions, which I have read and understand completely. To my knowledge, I have no medical condition or allergy which would preclude me from having a spray tan done. I have been honest and accurate about the   information that I have provided on this waiver. I take sole responsibility of any reaction or signs or symptoms I may develop, staining of clothing and/or personal belongings. I will contact Gotta Glow before my appointment if I have an specific requests or preferences.
